FINANCIAL ASSISTANCE FOR MEDICINES AND OUTSTATION TRAVEL

Need to Start?

Under this program parents and caregivers of the patients are provided financial assistance for the cost incurred by them on medicines and outstation. The beneficiaries of this program include:-

  • Single Mothers
  • Parents with disability
  • Patients from remote locations of the country bearing huge cost on outstation travel

The Programme team of Child Heart Foundation conduct the following activities to implement this programme

  • Identification of beneficiaries for the programme through referral or community outreach
  • Registration of beneficiaries
  • Adhering to the due diligence process which includes submission of following documents
  • Completed registration form
  • Income and Address Proof attested by local M.L.A
  • Attested copies of Aadhaar card
  • Copy of date of birth certificate
  • Photographs of the beneficiaries
  • Counselling session with patients
  • Submission of bills for medicines and outstation travel for reimbursements
  • Extending Financial assistance for medicines and outstation travel

  • At present 4 beneficiaries are supported under the CHF SAHYOG program
